Fermented Foods & Your Health: What the Science Actually Says

If you spend much time online, you've probably heard some version of this advice:

Eat fermented foods for your gut.

Sometimes the claim goes further. Fermented foods supposedly “heal the gut,” flood your digestive system with probiotics, reduce inflammation, improve immunity and fix everything from blood sugar to bloating.

There is real science behind some of the enthusiasm.

There is also a lot of marketing.

Foods such as yogurt, kefir, kimchi, sauerkraut, tempeh, miso and sourdough have been produced through fermentation for centuries. Modern research suggests that fermentation can change food in meaningful ways—altering its digestibility, nutrient availability, chemical composition and, in some cases, the microorganisms we consume.

But there is an important distinction:

Fermented does not automatically mean probiotic.

And fermented foods are not miracle foods.

Understanding what fermentation actually does gives us a much more useful—and much more interesting—picture of where these foods can fit into a healthy diet.

What Exactly Is a Fermented Food?

Fermentation is essentially controlled food transformation by microorganisms.

An international scientific consensus led by the International Scientific Association for Probiotics and Prebiotics (ISAPP) defines fermented foods as foods made through desired microbial growth and enzymatic conversion of food components.

In practical terms, bacteria, yeasts or fungi interact with carbohydrates and other compounds in the original food. During that process, they can produce organic acids, alcohols, gases and numerous other compounds.

Those changes can affect:

  • flavour

  • texture

  • preservation

  • digestibility

  • nutrient availability

  • carbohydrate structure

  • biologically active compounds

Fermentation is therefore much more than simply “adding good bacteria” to food.

In many cases, the microorganisms substantially change the food before it ever reaches your digestive system.

Fermented Foods Are Everywhere

Fermentation isn't a modern wellness trend.

Humans have used it for thousands of years to preserve food, improve flavour and make certain foods easier or safer to consume.

Common examples include:

  • yogurt

  • kefir

  • sauerkraut

  • kimchi

  • traditionally fermented pickles

  • miso

  • tempeh

  • natto

  • certain cheeses

  • kombucha

  • sourdough bread

Even coffee and chocolate involve fermentation during processing, although roasting later eliminates the microorganisms involved.

And this brings us to one of the most misunderstood aspects of fermented foods.

Fermented Does Not Mean Probiotic

The terms fermented food and probiotic food are often used as though they mean the same thing.

They don't.

A probiotic is a live microorganism that, when consumed in an adequate amount, has demonstrated a health benefit.

That definition requires more than simply discovering bacteria in a food.

Researchers need to know things such as:

  • which microorganism is present;

  • whether it is alive when consumed;

  • whether enough is present;

  • and whether research has demonstrated a health benefit from that microorganism.

A fermented food may contain live microorganisms without meeting those criteria.

And some fermented foods contain virtually no living microorganisms by the time we eat them.

Sourdough is a perfect example

Traditional sourdough begins with wild yeasts and lactic-acid bacteria.

Those microorganisms ferment the dough.

Then we put the loaf into a hot oven.

The baking process kills the vast majority of those microorganisms.

Sourdough is therefore a fermented food, but baked sourdough shouldn't automatically be described as a probiotic food.

That doesn't mean fermentation accomplished nothing.

Quite the opposite.

The benefit may come from what the microorganisms did to the food during fermentation, rather than from eating the microorganisms themselves.

What Can Fermentation Actually Do?

This is where the science gets interesting.

1. Fermentation can make some foods easier to digest

Microorganisms don't sit around doing nothing during fermentation. They metabolize components of the food.

One of the clearest examples is yogurt.

The bacterial cultures used to make yogurt help break down lactose, the naturally occurring carbohydrate in milk.

Research has consistently found that yogurt containing live cultures can improve lactose digestion and tolerance in people who have difficulty digesting lactose compared with milk. A systematic review identified multiple randomized controlled trials demonstrating this effect.

That doesn't mean everyone with lactose intolerance can consume unlimited yogurt without symptoms.

Tolerance varies.

But it illustrates an important principle:

Fermentation can partially perform digestive work before the food reaches you.

2. Fermentation can change nutrient availability

Plant foods naturally contain compounds known as phytates.

Phytate isn't inherently “bad”—and the internet often oversimplifies so-called antinutrients—but it can bind minerals including iron, zinc, calcium and magnesium and reduce their absorption.

Certain fermentation processes can reduce phytate levels.

That means fermentation may improve the bioavailability of some minerals in particular foods.

Again, this doesn't transform fermented foods into nutritional superfoods.

It simply means the nutritional properties of a food can change during fermentation. The scientific consensus on fermented foods identifies phytate reduction as one mechanism through which fermentation can alter nutritional value.

3. Fermentation creates new compounds

Microorganisms don't merely remove things from food.

They also create things.

Depending on the food and microorganisms involved, fermentation can generate organic acids, peptides and other biologically active compounds.

Some of these compounds may influence digestion, metabolism or microbial activity within the gastrointestinal tract.

This is an expanding field of research, and we shouldn't pretend every metabolite produced during fermentation has a clinically meaningful health effect.

But it helps explain why scientists are interested in fermented foods beyond the simplistic idea of “good bacteria.”

What About Your Gut Microbiome?

This is probably the biggest reason fermented foods have become popular.

Your digestive tract contains an enormous microbial ecosystem collectively known as the gut microbiota. The microbiome refers more broadly to these organisms and their genetic material and functions.

These microorganisms interact with digestion, metabolism, the intestinal environment and immune function.

That does not mean that every health problem is caused by an unhealthy microbiome—or that eating kimchi can fix it.

In fact, a 2026 ISAPP expert consensus highlighted just how loosely the term “gut health” is often used. The group proposed defining gut health more practically as normal gastrointestinal function without active gastrointestinal disease or gut-related symptoms that impair quality of life.

That's a useful reality check in a marketplace filled with products promising to “reset” or “heal” your gut.

Still, fermented foods are one dietary factor researchers are investigating.

And one particularly interesting human trial got a lot of attention.

The Stanford Fermented-Food Study

Researchers at Stanford conducted a randomized dietary intervention involving healthy adults who were assigned either a diet high in plant fibre or one high in fermented foods.

The fermented-food group consumed foods including yogurt, kefir, fermented cottage cheese, kimchi, fermented vegetables, vegetable brine drinks and kombucha.

Thirty-six participants completed the study.

As fermented-food intake increased, researchers observed an increase in gut microbial diversity.

They also detected reductions in several measured inflammatory proteins and changes in immune-cell activation.

That is fascinating.

It is not, however, proof that fermented foods cure inflammation.

The trial was small, involved healthy adults and studied a relatively intensive dietary intervention over a limited period. Its primary cytokine-response outcome did not change significantly, even though several secondary inflammatory measures did.

So the responsible interpretation isn't:

“Fermented foods fight inflammation.”

It's closer to:

Increasing fermented-food intake produced measurable changes in the microbiome and several immune markers in a small randomized human study, making this an interesting area for further research.

Less exciting for social media.

Much more scientifically accurate.

Different Fermented Foods Offer Different Things

Another mistake is treating “fermented foods” as though they're nutritionally interchangeable.

They're not.

Yogurt

Depending on the product, yogurt can provide:

  • protein

  • calcium

  • live cultures

  • improved lactose digestibility

Plain Greek-style yogurt can also provide substantial protein, making it useful for active adults.

Watch the added sugar in heavily sweetened products.

Kefir

Kefir is a fermented milk drink containing a community of bacteria and yeasts.

Like yogurt, it can provide protein, calcium and live microorganisms, depending on processing.

It can be consumed on its own or added to smoothies.

Sauerkraut

Traditional sauerkraut is fermented cabbage.

Unpasteurized refrigerated varieties may contain living microorganisms, while heat-treated shelf-stable products may not.

It can be an easy way to add fermented vegetables to meals.

One consideration: sauerkraut can be high in sodium.

Kimchi

Kimchi traditionally combines fermented vegetables with seasonings.

It can contribute vegetables and fermentation products to the diet and may contain living microorganisms when it hasn't been heat-treated.

Again, sodium content can be substantial.

Tempeh

Tempeh is particularly interesting because it's both fermented and a useful protein source.

Made from fermented soybeans, it provides protein, fibre and several micronutrients.

For someone looking to include more plant-based protein, tempeh can be useful regardless of whether they care about fermentation.

Miso

Miso is produced by fermenting soybeans, often with grains and koji cultures.

It adds considerable flavour to soups, sauces and marinades.

However, it can be high in sodium, and cooking or adding it to very hot liquids can reduce the number of living microorganisms.

Kombucha

Kombucha is fermented tea.

It has become almost synonymous with “gut health,” but the health claims surrounding it often run well ahead of the clinical evidence.

Some products also contain substantial added sugar, and fermentation can produce small amounts of alcohol.

Enjoy kombucha if you like it.

You don't need to drink it to have a healthy gut.

Sourdough

Traditional sourdough fermentation can alter the structure and chemistry of bread dough, potentially influencing starch digestion, organic-acid content and phytate degradation.

But sourdough still contains carbohydrate.

It can still raise blood glucose.

And because baking kills the microorganisms, its potential advantages don't depend on sourdough functioning as a probiotic.

That distinction matters.

What About Pickles?

Here's another surprise.

Not every pickle is fermented.

Many commercial pickles are simply cucumbers preserved in vinegar.

Traditional fermentation, by contrast, involves microorganisms fermenting the food—often in a salt brine.

Both can fit into a healthy diet.

But pouring vinegar over a cucumber doesn't automatically turn it into a fermented food. The ISAPP consensus specifically distinguishes vegetables preserved with vinegar from genuinely fermented vegetables.

Are Fermented Foods Better Than Fibre?

No—and that's the wrong contest.

Fibre-rich plant foods provide substrates that gut microorganisms can use. Fruits, vegetables, legumes, nuts, seeds and whole grains also provide vitamins, minerals and numerous phytochemicals.

Fermented foods potentially influence the intestinal environment through different mechanisms.

The Stanford study didn't prove fermented foods were “better” than fibre. In fact, researchers observed different biological responses to the two interventions, and responses to the high-fibre diet appeared to vary according to participants' baseline microbiomes.

A sensible eating pattern can contain both.

Think:

plants + fibre + adequate protein + minimally processed foods + some fermented foods you enjoy.

That's far more defensible than building your diet around whichever microbiome hack is trending this month.

Do You Need Fermented Foods Every Day?

There is currently no universally established number of fermented-food servings that everyone must eat each day.

And you certainly don't need to force down sauerkraut because someone on Instagram said your microbiome needs it.

A more practical approach is to introduce fermented foods gradually and use them as part of normal meals.

For example:

  • plain yogurt with berries and nuts;

  • kefir in a smoothie;

  • sauerkraut alongside eggs, potatoes or meat;

  • kimchi with rice, vegetables and a protein source;

  • tempeh in a stir-fry;

  • miso in soups or marinades;

  • traditional sourdough as your bread choice.

You don't need all of them.

Choose foods you enjoy and tolerate.

More Isn't Necessarily Better

This is particularly important if you suddenly decide to overhaul your diet.

Going from almost no fermented foods to several servings of kefir, kimchi, sauerkraut and kombucha every day may not make your digestive system particularly happy.

Some people may experience:

  • bloating

  • gas

  • abdominal discomfort

  • changes in bowel habits

Certain fermented foods can also contain substantial sodium, added sugar or histamine and other biogenic amines that may matter to susceptible individuals.

People with significant gastrointestinal disease, food allergies, immune compromise or medically prescribed dietary restrictions should seek individualized guidance rather than treating fermented foods as therapy.

Common Mistakes

Assuming every fermented food contains probiotics

It doesn't.

Some contain live microorganisms; others don't. And even the presence of live organisms doesn't automatically satisfy the scientific definition of a probiotic.

Assuming more bacteria must be better

Microbiology isn't that simple.

Different microorganisms have different functions, and health effects can be strain-specific.

Buying “gut health” products without reading the label

A product can be fermented while also containing large amounts of added sugar or sodium.

Evaluate the whole food, not just the marketing language on the front.

Treating fermented foods as medicine

Fermented foods may contribute to a healthy dietary pattern.

They are not replacements for appropriate medical treatment.

Ignoring the rest of the diet

Adding a tablespoon of sauerkraut to an otherwise poor dietary pattern isn't going to magically transform your health.

Diet quality is cumulative.

Who May Benefit Most?

Most healthy adults can experiment with fermented foods if they enjoy and tolerate them.

They may be particularly practical for people who:

  • want greater dietary variety;

  • want to include foods containing live cultures;

  • tolerate yogurt better than milk;

  • want additional plant-based protein options such as tempeh;

  • enjoy sourdough but want to understand what fermentation actually changes;

  • want to improve overall diet quality without relying on supplements.

The important phrase is as part of an overall healthy diet.

Practical Application: Start With Food, Not Supplements

You can spend a remarkable amount of money in the supplement aisle trying to “optimize” your microbiome.

For most healthy people, that's not where we'd start.

Before worrying about expensive probiotic capsules, powders and microbiome tests, look at the fundamentals.

Are you eating enough vegetables?

Fruit?

Whole grains?

Legumes?

Protein?

Fibre?

Are you drinking enough water?

Are you physically active?

Are you sleeping reasonably well?

Could you add one or two fermented foods you actually enjoy?

Those questions aren't as marketable as a $70 bottle promising “advanced gut optimization.”

They're much more useful.

Purpose matters more than hype.

You wouldn't build a good training program around one magical exercise.

Don't build your nutrition around one magical food either.

Fermented foods can absolutely have a place in a healthy eating pattern. Some provide live cultures. Some may improve digestibility. Some alter nutrient availability. Some are excellent sources of protein or other nutrients regardless of fermentation. And emerging research suggests certain fermented-food dietary patterns may influence the microbiome and immune function.

That's worth paying attention to.

But the bigger picture still matters more.

Adequate protein, plenty of minimally processed plant foods, sufficient fibre, appropriate energy intake, hydration and a dietary pattern you can maintain consistently will matter far more than whether you drink kombucha every afternoon.

And nutrition doesn't operate independently of everything else.

Strength training, cardiovascular fitness, daily movement, sleep, stress management and recovery all contribute to long-term health.

We aren't looking for hacks.

We're building capacity.

That's true in the gym, and it's true at the dinner table.

STOP EXERCISING. START TRAINING.

Are fermented foods probiotics?

Not necessarily. Some fermented foods contain live microorganisms, but the scientific definition of a probiotic requires demonstrated health benefits from defined live microorganisms consumed in adequate amounts. Fermented and probiotic are not interchangeable terms.

Is sourdough a probiotic food?

Generally, no. The microorganisms involved in sourdough fermentation are largely killed during baking. Fermentation can still change the nutritional and digestive properties of the bread before baking.

Which fermented food is best for gut health?

There isn't enough evidence to identify one universally “best” fermented food. Different foods provide different nutrients and microorganisms. Variety, tolerance and the quality of the overall diet matter.

Does yogurt really help with lactose intolerance?

After decades of coaching, I've watched nutrition advice go through the same cycles as fitness advice.

One food becomes the hero. Another becomes the villain. Everyone jumps on board, and a few years later we're talking about something completely different.

Fermented foods are interesting because there really is good science behind fermentation. But that doesn't mean everyone needs a refrigerator full of kimchi and kefir.

If you like yogurt, eat yogurt. If you enjoy sauerkraut, put some on your plate. If sourdough is the bread you prefer, great. If you've never tasted tempeh, try it and see what you think.

The goal isn't to collect as many “healthy” foods as possible.

It's to build an eating pattern that gives your body what it needs, supports your training, fits your lifestyle and is realistic enough that you're still doing it years from now.

That's the same lesson I've learned in the gym.

The basics aren't boring when they work.
